Washington Consumer Health Data Privacy Policy

How Insight K9 Solutions handles the limited handler health, accommodation, and service-dog task information needed to evaluate and provide requested training.

Insight K9 Solutions LLC (“Insight K9,” “we,” “us,” or “our”) maintains this separate policy for consumer health data covered by the Washington My Health My Data Act.

This page concerns consumer health data only. Our handling of other personal information is explained in our general Privacy Policy.

Section 01

Scope

This policy applies to consumer health data we collect through service-dog inquiries, booking and intake forms, communications, client records, and training services. Consumer health data is personal information linked or reasonably linkable to a person that identifies or permits an inference about that person’s past, present, or future physical or mental health status.

Health and behavior information about a dog is not consumer health data unless it also identifies, reveals, or permits an inference about the handler’s health status.

Section 02

Consumer health data we collect

We collect only the health-related information reasonably necessary to evaluate or provide the service a client requests.

Handler health and accessibility

Health or disability status, functional limitations, symptoms, injury or activity restrictions, accessibility needs, safety considerations, and related accommodation information that a client provides.

Service-dog tasks and assistance

Requested tasks, alerts, responses, handler goals, functional assistance needs, and related information that may reveal or permit an inference about a client’s health.

Service records

Booking or intake answers, relevant communications, task plans, assigned-trainer notes, and service records that contain or are linked to consumer health data.

Linked identifiers

Name, email address, telephone number, account or booking identifier, and technical identifiers when they are associated with a health-related inquiry, disclosure, or service-dog request.

Information we do not need

Please do not provide unrelated health details. If more information is needed to plan or safely provide a requested service, we will ask for it directly.

Sources and purposes

Sources

We may receive consumer health data from:

  • You or a person you authorize, through booking or intake forms, email, text message, telephone, documents, or in-person conversations.
  • An assigned trainer or other authorized personnel through relevant observations, communications, service notes, and task-planning records.
  • Our website, booking, communications, and recordkeeping systems when identifiers or activity records are linked to a health-related request.
  • Information reasonably inferred from the service-dog tasks, accommodations, or functional assistance you request.

Purposes

We collect and use consumer health data only as necessary to:

  • Evaluate whether and how we can provide the service you request.
  • Plan and provide requested service-dog task training, handler instruction, accommodations, and related support.
  • Communicate about your inquiry, appointments, training plan, safety considerations, and service records.
  • Associate information with the correct client, administer the requested service, and maintain relevant client and training records.
  • Protect the security and integrity of our systems and respond to consumer health data requests.

We do not use consumer health data for targeted advertising or unrelated marketing.

Section 04

Processors and disclosures

We do not sell consumer health data. We do not disclose consumer health data to third parties or affiliates for targeted advertising or for their own independent purposes.

Categories of consumer health data shared with third parties or affiliates: None under our current practices.

Authorized personnel, including an assigned trainer, may access only the information necessary to provide the service you requested. Service providers acting as our processors may process necessary consumer health data on our behalf and under our instructions. Processor categories include:

  • Website hosting, database, security, storage, and backup providers.
  • Booking, scheduling, and client-record management providers.
  • Email, SMS, telephone, and service-notification providers.
  • Payment-support providers when a service or transaction record is linked to consumer health data.

Categories of third parties with which we share consumer health data: None.

Specific affiliates with which we share consumer health data: None.

If we propose to collect or use an additional category of consumer health data, use consumer health data for an additional purpose, or share it in a way not described here, we will make the required disclosure and obtain any affirmative consent required before doing so.

Section 05

Your rights

Subject to the Washington My Health My Data Act, you may:

  • Confirm whether we collect, share, or sell consumer health data concerning you.
  • Access your consumer health data, including a list and active contact information for any third parties or affiliates with which it has been shared or sold.
  • Withdraw consent from future collection or sharing of your consumer health data.
  • Request deletion of your consumer health data.
  • Appeal a refusal to act on your request.
  • Exercise these rights without unlawful discrimination.
Section 06

Requests and appeals

You may submit a request at any time without creating a new account.

  1. Email Ryan@InsightK9Solutions.com with the subject Consumer Health Data Request.
  2. State whether you are requesting confirmation, access, withdrawal of consent, or deletion, and provide the name and contact information previously used with us. Do not include additional health details in the initial email.
  3. We will use commercially reasonable means to authenticate the request before disclosing or deleting information.

We respond without undue delay and within 45 days after receiving a request. When reasonably necessary, we may extend that period once by an additional 45 days and will provide notice and an explanation during the initial period. Information is provided without charge up to twice per consumer each year.

When deletion is requested, we will delete the covered data from our records and notify applicable processors and other recipients. Deletion from archived or backup systems may be delayed for no more than six months after we authenticate the request.

Appeals

To appeal a refusal, email Ryan@InsightK9Solutions.com with the subject Consumer Health Data Appeal. We will provide a written response within 45 days. If we deny the appeal, our response will explain how to submit a complaint to the Washington State Attorney General.

Section 07

Security

We restrict access to consumer health data to authorized personnel and processors that need it for the purposes described in this policy or to provide a service you requested. We maintain administrative, technical, and physical safeguards appropriate to the nature and volume of the consumer health data we handle.

Section 08

Changes and contact

We may revise this policy to reflect changes in our practices or legal requirements. The effective date at the top identifies the current version. Before collecting, using, or sharing an additional category of consumer health data or using it for an additional purpose not disclosed here, we will update this policy and obtain any affirmative consent required by Washington law.
